MEDICAL FORM INFORMATION

By law, all campers must have up-to-date medical forms on file in order to begin camp. A physician must have examined your child after September 1, 2023 to cover their entire camp experience. Please note that we require all forms be returned to us by June 1st.

Once registered, our team will follow up via email with all the necessary forms to fill in.

Please note which forms require the appropriate actions below:

  • Physician Form - Please have your child's physician fill out (or they can use their own “standard camp form”) - please return via email

  • Health History - to be filled out by a parent/guardian

  • Request for Administration of Medication - in order for our Nurse to administer Prescription Medication (if necessary), please print and have your child's physician fill out and return to us via email.

  • Over-the-Counter Medication - Please print and have your child's physician fill out and and return to us via email. NY State Law requires this form be filled out by your physician in order for our Nurse to dispense over the counter non-prescription medication to your child when needed.

Prescription and Non Prescription Medications

In order for our Nurse to administer any prescription or non-prescription medications to your child at any time during camp, the forms noted above must be filled out and signed by your physician. Campers should never carry any medications in their backpacks to camp. All medication should be sent to camp in its original bottle and dropped off at our Health Center before the camp season begins.

What happens if my child gets sick/injured at camp?

If your child becomes sick/injured at camp we will call you. Our philosophy regarding parent communication is very simple, the more the better! Our Nurse/Camp Director will contact you when your child is seen by our Nurse for anything and everything that we feel is important for a parent to know. If you are unavailable, we will call the local emergency contacts that you have provided. In the event of a medical emergency and the parent and emergency contacts cannot be reached, the Nurse in conjunction with the Camp Directors will decide how to proceed.
